Many people take the opportunity to travel, either during the summer vacation, or because it is a requirement of a student’s course. 

Travel is a great opportunity to experience different cultures and environments. Market Rasen Surgery want to ensure that you remain healthy and protected from important diseases during any trips abroad.

We have developed a comprehensive travel service for registered patients of the practice. Unfortunately, our travel service is not available to individuals not registered with the practice.

Please note some travel vaccinations require a fee.

It is advisable to make us aware of your travel plans as far in advance as possible as some vaccination schedules can take several weeks to complete. We suggest contacting the surgery at least 3-6 months before you travel.

Ask the reception for a travel risk assessment form or you can download it via the button below. The form includes information such as locations to be visited, how long for, type of travel and accommodation, past medical history and any previous vaccinations. It is important to include ALL previous vaccinations on the form. Please don't assume we will already have them recorded. 

Once you have completed the form, bring it back to the surgery and hand it in to reception. The nurse will assess your travel plans and medical information included on the form and, if necessary, contact you to commence any vaccination courses. If the vaccinations you require incur a fee, the nurse will make you aware of this but may not be able to give you a confirmed amount as the cost of vaccines often change. Once you have confirmed you would like to go ahead with the vaccination course, our admin team will generate an invoice which includes details of the charges. 

Some vaccination courses can take up to 6 months so it is very important that you complete the travel form and bring it back to us as soon as you can.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
