We have to restrict the area we cover in order for our doctors and nursing staff to be able to manage all requests for home visits. If you move out of this area, we will ask you to register with a practice closer to where you have moved. Please use the "Find a GP" tab at the top of this page to find your nearest GP Practice.
In order to register, you will need to complete a registration forms and provide us with your NHS number (shown on your NHS medical card) and your previous doctor’s details. If you are on any repeat medication, please bring in your repeat slip.
Your registered GP will be Dr K Thomas.
You will need to bring identification, the following documentation is acceptable:
- Birth certificate
- Marriage certificate
- Medical card
- Driving licence
- Passport
- Paid utility bill
- National insurance card
If you are over the age of 16 you will be invited to attend for a new patient health check, initially this is a 20 minute appointment with the Health Care Assistant.
